Abstract:Objective To develop a hospice shared-care case management programme based on the PEPPA model, with the goal of providing reference and guidance for the practice of hospice shared-care case management. Methods An initial draft of the hospice shared-care case management service plan was constructed based on the steps defined in the PEPPA model, including patient group identification and care plan development, stakeholder identification, assessment of care needs, identification of priority issues and goals, and definition of the role of the case manager within the hospice shared-care programme. Then the plan was reviewed by 28 experts through two rounds of expert consultations. Results A 14-step,34-task hospice shared-care case management programme was developed. The variation coefficients of the primary indicators ranged from 0.085 to 0.218,and the secondary indicators ranged from 0.064 to 0.237.The Kendall's concordance coefficients for the primary and secondary indicators were 0.406 and 0.412,respectively.Conclusion The hospice shared-care case management protocol developed based on the PEPPA model provides a clear approach for case management in hospice care and has good potential for widespread adoption.
