Application of hospital-community-family ternary linkage continuity of care model in patients with lower extremity venous ul-cers

Abstract:Objective To explore the impact of the continuous nursing model based on the tripartite linkage of hospital-community-family on the relevant indicators in patients with lower extremity venous ulcers,including the self-management ability,self-efficacy,ulcer cure rate,and the consultation rate between hospitals and communities.Methods A total of 123 patients with lower extremity venous ulcers in a certain community hospital from August 2022 to December 2023 were selected as the research objects.The patients were divided into the observation group(61 cases)and the control group(62 cases)according to the random number table method.The patients in the control group adopted the traditional community wound care,while the patients in the observation group received the continuous nursing model based on the tripartite linkage of hospital-community-family.The patients'self-management behaviors and self-efficacy were evaluated before the intervention and 3 months and 6 months after the intervention.The ulcer cure rate and the consultation rate between hospitals and communities were compared 6 months after the intervention.Results Three months and 6 months after the implementation of the continuous nursing mod-el,the self-management ability and the self-efficacy of patients in the observation group were higher than those in the control group(P<0.05).After 6 months of intervention,the ulcer cure rate of patients in the observation group was higher than that of the control group(P<0.05),and the consultation rate between the hospital and the community was lower than that of the control group(P<0.05).Conclusion The continuous nursing model based on the tripartite linkage of hospital-community-family can effectively improve patients'self-management ability,self-ef-ficacy,and the cure rate and reduce the consultation rate between hospitals and communities.It is of great significance for improving the treat-ment effect of patients with lower limb venous ulcers.
