Development and validation of risk assessment scale for unplanned extubation of transnasal gastrointestinal tube in adults

Abstract:Objective To develop an adult nasoenteric tube unplanned extubation risk assessment scale and evaluate its reliability and validity.Methods Literature review and 2 rounds of expert consultation were used to construct the risk assessment scale of unplanned extu-bation of adult transnasal gastrointestinal tube.A total of 120 patients from the department of emergency surgery,gastroenterology and general surgery of a class A tertiary hospital in Qinghai Province were selected by convenience sampling method to screen the items of the scale and test the reliability and validity.Critical ratio method was used to determine patients'risk levels(low,medium,high)for unplanned extubation.Results The adult risk assessment scale for unplanned extubation of transnasal gastrointestinal tube contained 4 dimensions,namely physio-logical factors,psycho-psychological factors,environmental factors and other factors,with 18 first-level items and 48 second-level items.Four common factors were extracted through exploratory factor analysis,and the cumulative variance contribution rate was 65.343%.The absolute value of factor load of each item ranges from 0.518 to 0.862.The content validity index at the scale level(S-CVI)was 0.940,and the content validity index at the item level(I-CVI)was between 0.90~1.00.The Cronbach'α coefficient of the whole scale was 0.786,and the broken half reliability was 0.724.The total scale score ranges from 0 to 94 points,with 0~31 points indicating low risk,>31~49 points indicating me-dium risk,and>49 points indicating high risk.Conclusion The risk assessment scale of unplanned extubation for adult transnasal gastroin-testinal tube has good reliability and validity,and can be used as an auxiliary tool to identify and predict the risk of unplanned extubation.
