Construction and validation of reliability and validity of a risk warning scoring scale for the deterioration in critical neurosur-gical patients

Abstract：Objective To develop a risk warning scoring scale for detecting deterioration in critical neurosurgical patients using the Delphi technique,and to evaluate its reliability and validity.Methods Based on literature review,group discussion,and Delphi method,an initial version of risk warning scoring scale for deterioration in critical neurosurgical patients was developed.A total of 400 critical patients ad-mitted to neurosurgery departments of two tertiary comprehensive hospitals in Sichuan and Yunnan provinces were selected via cluster sam-pling.Items were refined using item analysis,and the scale's reliability and validity were systematically evaluated.Results Two rounds of Delphi consultation were completed by 28 national neurosurgery experts,with 100%effective questionnaire response rates.The authority coeffi-cients of expert were 0.86 and 0.88,respectively.The Kendall's W of expert on indicators were 0.306 and 0.332,respectively(both P＜0.001).The overall content validity S-CVI of the scoring table was 0.998.Exploratory factor analysis extracted four common factors,accounting for 65.606%of the cumulative variance.Confirmatory factor analysis confirmed the model fit(x2/df=1.491).Cronbach's α for internal consisten-cy reliability of the scoring scale was 0.839.The inter rater reliability Kendall's W for two hospitals were 0.987 and 0.997(both P＜0.001).Conclusion The final risk warning scoring scale for deterioration in critical neurosurgical patients includes 4 dimensions and 14 indicators,with good reliability and validity,and practicality and feasibility.It provides an early assessment tool for clinical medical staff to identify the deterioration of critical patient conditions,and supports the development of electronic risk warning system.
