A study on the status and influencing factors on the patient activation levels of elderly people with hypertension in the commu-nity

Abstract:Objective To explore the current situation and influencing factors of the patient activation levels among elderly people with hypertension in the community.Methods A total of 323 elderly people with hypertension in the community were surveyed using the general information questionnaire,Patient Activation Measure-13,Self-Efficacy Scale for Hypertension Patients,Hypertension Patients Self-Manage-ment Behavior Rang Scale,Health Literracy Management Scale and Patient Health Questionnaire-9.Results The patient activation score of elderly people with hypertension in the community was 60.02(39.08,75.25)points,the self-efficacy score was(41.54±7.61)points,the self-management behavior assessment score was(106.29±19.39)points,the health literacy score was(80.02±14.27)points,and the de-pression score was 3.00(1.00,5.00)points.The patient activation levels of elderly people with hypertension in the community was positively correlated with self-efficacy,self-management behavior,and health literacy(r1=0.499,r2=0.437,r3=0.306,all P<0.01),and negatively correlated with depression(r=-0.286,P<0.01).The occupation before retirement,alcohol consumption,duration of hypertension,risk level of hypertension,self-efficacy,self-management behavior,health literacy and the degree of depression were the main influencing factors of patient activation levels(P<0.05).Conclusion The elderly people with hypertension in community demonstrate the intermediate activation levels with significant heterogeneity across activation stages.Healthcare staff should implement targeted multidimensional interventions focusing on subgroups exhibiting risk predictors including pre-retirement unemployment,alcohol consumption,prolonged hypertension duration,advanced disease severity,and comorbid depressive symptoms.By strategically enhancing self-efficacy cultivation,self-management capacity building,and health literacy empowerment,these interventions can effectively elevate patient activation levels,thereby to improve clinical prognosis and quality of life.
