Incidence and influencing factors of fear of cancer recurrence in postoperative patients with digestive system cancers:a Meta-analysis

Abstract：Objective To systematically evaluate the incidence and influencing factors of fear of cancer recurrence(FCR)in postoper-ative patients with digestive system cancer.Methods Through computer retrieval of Chinese and English databases such as Cochrane Librar-y,CINAHL Complete,PubMed,Embase,Web of Science,China National Knowledge Infrastructure,Wanfang Data,VIP and CBM,the relevant literatures on the incidence and the influencing factors of FCR in postoperative patients with digestive system cancer were searched from in-ception to October 20,2024.Stata 16.0 software was used for systematic review.Results A total of 28 papers with 6 523 Chinese postopera-tive patients with digestive system cancer were included in the analysis.The Meta-analysis showed that the incidence of FCR in postoperative patients with digestive system cancer was 47.60％.The subgroup analysis showed that the incidence of postoperative FCR in patients with colo-rectal cancer,hepatocellular carcinoma,and gastric cancer was 32.76％,57.94％,and 56.73％,respectively,which had statistically significant difference(x2=170.12,P＜0.001).The incidence of FCR in postoperative patients with digestive system cancer was 42.00％,60.92％,40.86％,57.88％,and 51.27％in northeast China,Central China,East China,South China,and North China,which had statistically significant difference(x2=25.19,P＜0.001).The main influencing factors included cancer stage(r=0.22),positive coping(r=-0.36),negative coping(r=0.38),anxiety and depression(r=0.34),illness uncertainty(r=0.25),hope level(r=-0.20),self-efficacy(r=-0.30),and psychologi-cal resilience(r=-0.45).Conclusion The incidence of FCR in patients with digestive system cancer after surgery is high which is affected by different cancer types and regions.Cancer stage,coping style,anxiety and depression,illness uncertainty,hope level,self-efficacy and psy-chological resilience are the main influencing factors.
