The analysis of current situation and influencing factors of decision-making conflict faced by proxy decision-makers on ad-vanced stage cancer patients

Abstract:Objective To investigate the current situation of decision-making conflict faced by proxy decision-makers on advanced stage cancer patients and analyze its influencing factors,meet the information needs of proxy decision-makers,and improve their decision-making and coping abilities.Methods The proxy decision-makers of advanced cancer patients treated in a cancer hospital in Harbin from December 2023 to February 2024 were selected as the study subjects by convenience sampling method.General data questionnaire,Decision conflict Scale,Deci-sional Fatigue Scale,Wake Forest Physician Trust Scale and Social Support Rating Scale were used to investigated.Results The decision di-lemma score of the proxy decision-maker for advanced cancer patients was 30.00(21.50,38.50),the decision fatigue score was 13.00(6.50,18.00),the physician trust score was(30.75±7.31),and the social support score was(36.48±8.31).Pearson correlation analysis showed that the total score and scores of various dimensions of decision-making conflict for advanced stage cancer patients were positively correlated with the total score of decision fatigue(P<0.01),and negatively correlated with the total score and scores of physician trust,social support and various dimensions(P<0.01).The results of multiple linear regression analysis showed that educational level,medical back-ground,decision fatigue,physician trust,and social support were the main influencing factors of decision-making conflict for proxy decision-makers in advanced cancer patients(P<0.01),which could explain 43.7%of the total variation.Conclusion The decision conflict of proxy decision makers in advanced cancer patients is generally at a moderate level,and the education level,medical background,decision fatigue,physician trust and social support of proxy decision makers affect the proxy decision-making dilemma.It suggests that nursing managers should take effective measures to help proxy decision makers to better understand the problems and choose solutions,improve the quality of decision making,and reduce the decision-making pressure and conflict of surrogate decision makers.
