Abstract：With the acceleration of the aging process in China,acute respiratory distress syndrome,which had a high incidence and mortality rate among the elderly population,not only imposed a heavy burden on patients and their families,but also highlighted the urgency of transitional care interventions.The article outlined the concept,origin,and development of transitional care,and reviewed the forms of transi-tional care for elderly patients with acute respiratory distress syndrome,including discharge readiness services,case management and telemedi-cine care.There were difficulties in current nursing work,such as mechanical defect in social support systems,systemic disconnections in health education and diminished efficacy in the nursing execution process.In response to these problems,strategies are proposed to enhance the effectiveness of social support systems,improve the coherence and effectiveness of health education and increase the efficiency and quality of nursing execution.It provided a basis for constructing a transitional care model suitable for elderly patients with acute respiratory distress syndrome in China and promoted the rehabilitation of patients.
