The construction and preliminary application of an intervention program for engagement in pain management on patients un-dergoing total knee arthroplasty

Abstract:Objective To construct an intervention program of engagement in pain management for patients with total knee arthroplasty(TKA)and test the preliminary application effect.Methods Based on the Patient Health Engagement Model,a draft program including cog-nitive,emotional,and behavioral dimensions was developed by literature review and qualitative interview on 12 patients.A Delphi method with 2 rounds was conducted including 16 experts in both rounds of expert consultation that resulting in a final version.A total of 15 patients and 17 nurses at the department of joint orthopaedic surgery in a Grade A tertiary hospital in Guangzhou were recruited for preliminary application and evaluation of the program.Results The response rates for two rounds of expert consultation were 100%,with expert authority coefficients of 0.953 and 0.950,Kendall's concordance coefficients of 0.216 and 0.132(P<0.001)respectively.The final version of the program includ-ed 3 dimensions with 8 secondary items and 24 tertiary items.After implementation,scores of patients'participation in pain treatment and sat-isfaction with pain treatment were(3.00±1.00)and(9.00±1.00)respectively,the average percentage of pain relief was 73.33%.Nurses'scores of the program's specificity,feasibility,applicability,and clinical significance were all over score 4.5.Conclusion The intervention program for engagement in pain management on TKA patients is scientific,targeted and feasible,which can provide reference to improve pa-tients'participation in pain management.
