Construction of hierarchical management evaluation index system for chronic diseases under the"hospital-community"link-age model

Abstract：Objective To construct an evaluation index system for hierarchical management of chronic diseases under the"hospital-community"linkage model by Delphi method.Methods Based on the Donabedian model,the hierarchical management evaluation index sys-tem of chronic diseases under the"hospital-community"linkage model was preliminarily formulated by literature review.On the basis,the Delphi method was used to further refine and determine the index system after two rounds of questionnaire consultation and analysis of 15 experts.Results Through two rounds of expert consultation,the positive coefficients of the two rounds were 100％,the authority coeffi-cients were 0.834 and 0.874,and the overall coordination coefficients were 0.374 and 0.264,respectively(P＜0.05).Finally,the hierarchical management evaluation index system of chronic diseases under the"hospital-community"linkage model included 3 primary indicators,12 secondary indicators and 48 tertiary indicators.Conclusion The constructed evaluation index system in the study has great scientificity,reliability and practicability,and provides a reference for the hierarchical management of chronic diseases under the"hospital-community"linkage model.
