Current situation of ophthalmic medication deviation in elderly patients during the hospital-home transition period and its in-fluencing factors analysis

Abstract:Objective To analyze the incidence,types,causes and influencing factors of medication deviation in elderly ophthalmic pa⁃tients during the transition period from hospital to home.Methods A total of 235 elderly patients undergoing eye surgery in a Grade-A eye hospital in Qingdao from April 2022 to July 2023 were investigated using convenience sampling method.Baseline data of the patients were col⁃lected before discharge.Within one week after discharge,Medication Discrepancy Tool,Self-efficacy for Appropriate Medication Use Scale,Drug Regimen Unassisted Grading Scale and Morisky Medication Adherence Scale were used to conduct the investigation.Results The inci⁃dence of medication deviation among 235 elderly patients within one week after discharge was 34.47%.The most common type of deviation was"decrease in the medication frequency",followed by "incorrect medication administration".Patient-derived medication bias accounted for 61.28%and iatrogenic medication bias accounted for 38.72%.Logistic regression analysis showed that living condition,frequency of daily medication use,independent medication use ability,and medication compliance were the influencing factors of medication deviation in elderly patients.Conclusion There is a high incidence of medication deviation in elderly ophthalmic patients within one week after discharge.Medi⁃cal staff should pay attention to medication deviation management for elderly ophthalmic patients and actively cooperate with pharmacists to provide discharge medication guidance for patients and their families,ensuring the safety of medication for elderly patients.
