Construction and efficiency of early warning model for intracranial hemorrhage after microvascular decompression

Abstract:Objective To construct an early warning model for intracranial hemorrhage after microvascular decompression in patients with functional neurological diseases.Methods From January 1,2018 to June 30,2022,2286 patients undergoing microvascular decompres-sion in the department of neurosurgery of a tertiary hospital in Jiangsu were selected as the research subjects by convenience sampling meth-od,and they were divided into the no bleeding group and bleeding group based on whether they had postoperative bleeding.The clinical data of the two groups were collected for single factor and multi-factor analysis,to screen out the risk factors for intracranial hemorrhage after micro-vascular decompression.Based on this,an early warning model was constructed and the prediction effect of the model was verified.Results Among the 2286 patients,41 had bleeding,with a bleeding rate of 1.79%.History of smoking,history of alcohol,carotid artery ste-nosis,small posterior fossa volume and postoperative systolic blood pressure were independent risk factors for intracranial hemorrhage after mi-crovascular decompression in patients with functional neurological diseases(P<0.05),and the area under the ROC curve was 0.947(95%CI:0.897,0.997).Conclusion Patients with a history of smoking,history of alcohol,carotid artery stenosis,small posterior fossa vol-ume and postoperative high systolic blood pressure are at greater risk of intracranial hemorrhage after microvascular decompression.The early warning model can provide a basis for the observation and prevention of intracranial hemorrhage,suggesting that nurses should pay attention to the early warning and prevention of intracranial hemorrhage complications in clinical work.
