Study on the influencing factors of enteral nutrition feeding intolerance in critically ill patients in neurology department at dif-ferent time periods

Abstract:Objective To explore the occurrence time and influencing factors of enteral nutritional feeding intolerance(FI)in critical ill patients in neurology department.Methods A retrospective analysis was performed on 426 critically ill patients receiving enteral nutrition in neurology department in a hospital from September 2021 to September 2022.The occurrence of FI during the duration of enteral nutrition was collected,and the corresponding occurrence time was recorded,and the factors influencing the duration of different enteral nutrition intol-erance were analyzed by Cox proportional risk regression model based on whether FI occurred and the time dependent.Results The inci-dence of FI in critically ill patients in neurology was 47.65%,and the median time of occurrence was 3(1,7)days.The Cox proportional haz-ards regression model showed that mechanical ventilation,mild hypothermia therapy,bedhead elevation and interrupted enteral nutrition sup-port had statistically significant effects on the occurrence time of FI during enteral nutrition treatment(P<0.01).When FI occurred at 1 and 3 d,mild hypothermia therapy,bedhead elevation and interrupted enteral nutrition support were the influencing factors.When the FI occurred at 4~7 days,bedhead elevation and interrupted enteral nutrition support were the influencing factors.Conclusion The incidence of FI is high and the time is scattered during enteral nutrition treatment in critically ill patients in neurology department.Nursing staff can focus on pa-tients with mild hypothermia treatment and carry out bedhead elevation and interrupted enteral nutrition support to reduce the risk of FI within 3 days.Patients with enteral nutrition at 4~7 days should mainly use bedhead elevation and interrupted enteral nutrition support to reduce the risk of FI.
