Effect of transitional care interventions on family caregivers for patients with chronic heart failure

Abstract：Objective To explore the effect of transitional care interventions on family caregivers for patients with chronic heart failure(CHF).Methods Two hundred and forty eligible family caregivers for patients with CHF were selected using convenience sampling,and e-qually divided into a control group and an experimental group using a table of random numbers.CHF patients cared by family caregivers in the control group and experimental group received regular nursing,and regular nursing plus transitional care interventions(designed and imple-mented by a transitional care group),respectively.Then the care burden,social support level and nursing satisfaction of family caregivers in the two groups were compared.Results After intervention,family caregivers in the experimental group had lower level of care burden compared with those in the control group(P＜0.01).Moreover,they had higher levels of social support(P＜0.01)and nursing satisfaction(P＜0.01).Conclusion The transitional care model intervention reduced the level of care burden,and improved the levels of social support and nursing satisfaction in family caregivers for patients with CHF.
