A study on the status and influencing factors of participation in advance care planning in surrogate decision-makers for pa-tients with advanced cancer

Abstract:Objective To investigate the status and associated factors of participation in advance care planning(ACP)in surrogate decision-makers for patients with advanced cancer.Methods Three hundred and seventy-five surrogate decision-makers of patients with ad-vanced cancer in a cancer hospital were sampled conveniently to participate in a survey using the general information questionnaire,Advance Care Planning Engagement Survey for Surrogate Decision Makers(ACPES-SDM),Anticipatory Grief Scale(AGS)and Decisional Conflict Scale(DCS).Results The mean ACPES-SDM score of the surrogate decision-makers was(2.99±0.60).Multiple linear regression analysis showed that education background,experience of caring for terminal patients,AGS score and DCS score were main factors associated with the participation of surrogate decision-makers in ACP(P<0.01).Conclusion The level of ACP involvement of surrogate decision-makers for advanced cancer patients is at a moderate level.To further improve this,it is recommended that nurses provide medical information support for surrogate decision-makers with low education level and high levels of anticipatory grief and decisional conflict,and with no experience of ca-ring for terminal patients.Moreover,nurses should provide decision-makers with decision aids and strengthened publicity of ACP to improve their awareness of palliative treatment and hospice care.
