Application of family doctor team service model based on the Omaha system in the continuous nursing care of diabetes melli?tus

Abstract:Objective To explore the effect of family doctor team service model based on the Omaha system in application of the con?tinuous nursing care of diabetes mellitus.Methods Totally,200 cases of patients with diabetes mellitus hospitalized from December 2015 to December 2016 in Futian District Hospital of Shenzhen city and lived in Futian District were selected as the study objects.The patients were divided into the observation group and the control group,with 100 cases of each group.Patients in the observation group were received continu?ous nursing care based on the Omaha system.While patients in the control group were followed up routinely.Two group patients'body mass in?dex,fasting blood glucose,2 hours postprandial blood glucose and glycosylated hemoglobin were measured before intervention and 6 months af?ter intervention,and the quality of life of patients with diabetes mellitus were assessed by using diabetes specific quality of life scale. Results There were no significant differences in body mass index(BMI),fasting blood glucose,2 hours postprandial blood glucose and gly?cosylated hemoglobin levels between the two groups before intervention(P>0.05),and both groups improved significantly after intervention and the observation group was significantly better than the control group(P<0.05).There was no significant difference in quality of life be?tween the two groups before intervention(P>0.05).They were improved after intervention and the observation group was significantly better than that of the control group(P<0.05).Conclusion The application of family doctor team service model based on the Omaha system in the continuous nursing care of diabetes mellitus is helpful to control the blood glucose and improve patients'quality of life.
