Effect of community home visits based on Transtheoretical Model on the compliance of actively participating in functional exercise for elderly patients with stroke

Abstract:Objective To explore the effect of community family visits based on Transtheoretical Model on the compliance of actively participating in functional exercise for elderly patients with stroke.Methods Totally,288 elderly patients with stroke were divided into two groups based on discharge number,and the single number group was intervention group as the double number was control group.Control group received routine family visits,on the basis of which intervention group carried out community family visits strategy based on Transtheoretical Model,which included evaluating patients'behavior change stage and giving matched behavior intervention.After intervention,to compare patients'behavior and compliance of functional exercise and self-care ability between two groups.Results The intervention group had higher scores of behavior and compliance of functional exercise (P < 0.01),and higher score of self-care ability (P <0.01) than those the of control group.Conclusion Community family visits based on Transtheoretical Model can improve patients'compliance of actively participating in functional exercise and self-care ability.
