Abstract：The paper reviewed the status of remote intervention in chronic disease continuous nursing,including the intervention models and group,and analyzed the intervention effect of remote intervention in chronic disease continuous nursing,then the paper put forward the envision improvements,so as to provide suggestions and references for developing the chronic disease continuous nursing remote intervention.
