Guided Care Model in the management of chronic diseases in the elderly

Abstract:Objective To explore the effect of Guided Care nursing model in the management of chronic diseases in the elderly.Methods Totally,160 elderly patients were divided into intervention group and control group,control group received conventional nursing and health education while intervention group received Guided Care program.After 6 months,the daily living ability,family function,social support scores were compared between two groups.Results Intervention group had higher scores than control group in the daily living ability,family function and social support (P < 0.01 or P < 0.05).Conclusion The implement of Guided Care for elderly patients with chronic diseases can improve their daily life ability,strengthen family function and social support,and improve the quality of life.
