Effect of standardizing the follow-up process for recovery on regulating cardiac rehabilitation follow-up procedure in patients with chronic heart failure

Abstract:Objective To investigate the effect of standardizing the follow-up process for recovery on regulating cardiac rehabilitation follow-up procedure in patients with chronic heart failure.Methods Totally,102 patients with chronic heart failure were divided into control group and intervention group.The control group received routine rehabilitation instruction and follow-up after discharge.The intervention group were followed by a manual based on clinical nursing pathway management model.Two groups were evaluated by Chronic Disease Self-efficacy Scale and Heart Failure Quality of Life Scale before intervention and at 6 weeks,12 weeks and 24 weeks after intervention.Results The scores of the self-efficacy and quality of life in intervention group were higher than those of control group (P < 0.01),which increased gradually over time (P < 0.01).Conclusion To standardize the follow-up process for recovery can improve patients' compliance,significantly enhance self-efficacy and enhanethe quality of life.
