Abstract：The paper summarized the model of continuing care in the aged with hip fracture,included family visit,telephone follow-up,outpatient follow-up and remote management,and expounded the basic contents of continuing care in the aged with hip fracture.Meanwhile,it enumerated the nursing value about several continuing care model in the aged with hip fracture,in order to provide references for the research.
