Mitoxantrone hydrochloride liposome combined with pegaspargase in the treatment of primary extranodal NK/T-cell lymphoma

Abstract:Objective To evaluate the efficacy and safety of mitoxantrone hydrochloride liposome combined with pegaspargase chemotherapy regimen on the treatment of primary extranodal natural killer(NK)/T-cell lymphoma(ENKTL).Methods Twenty-one patients with primary ENKTL were included from September 2020 to December 2023.They were treated with chemotherapy regimen of mitoxantrone hydrochloride liposome combined with pegaspargase.Efficacy was evaluated at the end of 2 cycles.Those achieving complete remission(CR)or partial remission(PR)were continuously given the chemotherapy regimen,and early-stage ENKTL patients were managed by chemotherapy for 4 cycles followed by local radiotherapy,and advanced patients were given chemotherapy for 4-6 cycles.Efficacy was re-examined,and all patients were followed up.The primary endpoint was the overall response rate(ORR),and secondary endpoints were disease control rate(DCR),overall survival(OS),overall progression-free survival(PFS),and safety outcomes.Results At the end-of-treatment,the cohort of ENKTL patients achieved 70%of CR,95%of ORR,and 100%of DCR.No significant difference in the short-term efficacy between subgroups was detected by the Chi-square test(P>0.05).The median overall follow-up time was 29 months,with the 2-year OS and PFS of 85.71%and 71.11%,respectively.Univariate prognostic analysis showed that tumor staging and the Prognostic Index of Natural Killer lymphoma with Epstein-Barr virus(PINK-E)were influencing factors for OS;and B symptoms,PINK-E score,primary lesions,and staging were the influencing factors for PFS.No relevant prognostic influences were shown by COX regression multivariate analysis.The main toxic side effects were bone marrow suppression,gastrointestinal reactions,and skin pigmentation mostly graded into stage 1-2,which could be tolerated after symptomatic treatment.Conclusion Mitoxantrone hydrochloride liposome combined with pegaspargase regimen is clinically effective in primary ENKTL with tolerable side effects and a high safety profile for clinical references,serving as a novel treatment modality for ENKTL.
