The short-term efficacy of anlotinib combined with TP chemotherapy in patients with stage Ⅳ non-small cell lung cancer and the regulatory role of the VEGF/MAPK/NF-kB signaling pathway

Abstract:Objective To investigate and analyze the short-term curative effects of anlotinib combined with TP chemotherapy(docetaxel and cisplatin)in patients with stage Ⅳ non-small cell lung cancer(NSCLC)and the regulatory role of the vascular endothelial growth factor(VEGF)/mitogen-activated phosphorylated kinase(MAPK)/nuclear factor kappa B(NF-kB)signaling pathway.Methods A total of 89 patients with stage Ⅳ NSCLC admitted in Zhangjiakou First Hospital from April 2022 to April 2023 were selected.They were divided into the control group(40 cases)and observation group(49 cases)according to different treatment plans.Patients in the control group received TP chemotherapy and those in the observation group were treated with anlotinib combined with TP chemotherapy.Both groups completed at least 2 cycles of treatment.The short-term and long-term curative effects,the changes of peripheral blood T lymphocyte subsets,the changes of serum VEGF,MAPK and NF-κB levels,and the toxic and side effects of chemotherapy were compared between the two groups.Results The clinical control rate of short-term treatment in the observation group was significantly higher than the control group(77.55%vs 57.50%,P<0.05).CD3+,CD4+,and CD4+/CD8+were significantly reduced after treatment in both groups(P<0.05),which were significantly higher in the observation group than the control group(P<0.05).The change in the percentage of CD8+cells before and after treatment was comparable in both groups(P>0.05).Serum VEGF,MAPK and NF-κB were significantly reduced after treatment in both groups(P<0.05),which were significantly lower in the observation group than the control group(P<0.05).Most of adverse events were grade 1-2,and the incidence of grade 1-2 and grade 3-4 adverse events was comparable between groups(P>0.05).Survival was significantly better in the observation group than the control group(P<0.05).Conclusion Anlotinib combined with TP chemotherapy in the treatment of advanced NSCLC can effectively improve the short-term efficacy,long-term survival prognosis and immune function by inhibiting the VEGF/MAPK/NF-κB signaling pathway.It is safe and worthy of clinical promotion.
