Evaluation of the effect of meropenem on cerebrospinal fluid penetration in meningitis patients after neurosurgery

Abstract:Objective To investigate the effect of meropenem on cerebrospinal fluid(CSF)penetration in meningitis patients after neurosurgery.Methods A total of 120 meningitis patients treated from 2021 to 2024 were compared.Meropenem was administered intravenously according to the schedule of 2g/8h,1g/8h,1g/6h.During the fourth administration,both blood and CSF samples were taken at a predetermined time point.The content of meropenem was determined by high-performance liquid chromatography(HPLC).Results The peak concentrations of meropenem in blood and cerebrospinal fluid were(43.2±5.3)and(2.4±0.3)mg/L in the 2g/8h group,(28.9±2.7)and(1.2±0.2)mg/L in the 1g/8h group,and(31.5±3.4)and(1.6±0.2)mg/L in the 1g/6h group,respectively.The maximum CSF permeability was 17.6%±7.3%,14.3%±1.7%and 30.9%±24.2%,respectively.Conclusion CSF permeability of meropenem at 1g/6h and 2g/8h is higher than 1g/8h.A higher dose of meropenem given in a shorter dosing interval is more effective in removing pathogens.
