Clinical value of coagulation function indicators in predicting the short-term prognosis of sepsis

Abstract:Objective To investigate the relationship between coagulation function indicators and 28-day mortality of sepsis,and to build a nomogram.Methods Clinical data of 130 sepsis patients admitted from July 2022 to December 2023 were retrospective collected.They were divided into the death group(41 cases)and survival group(89 cases)according to the 28-day mortality.The prediction factors for 28-day mortality were analyzed by Cox regression,and a nomogram was established and verified for its performance.Results Multivariate Cox regression demonstrated that,the Sequential Organ Failure Assessment(SOFA)scores(hazards ratio[HR]=1.540,95%confidence interval[CI]=1.297-1.828),D-dimer(HR=0.931,95%CI=0.892-0.972),thrombin time(HR=1.198,95%CI=1.073-1.337),prothrombin time(HR=1.169,95%CI=1.029-1.328),activated partial thromboplastin time(HR=1.377,95%CI=1.128-1.681),fibrinogen(HR=1.030,95%CI=1.006-1.054),and sepsis-associated coagulopathy(HR=0.439,95%CI=0.202-0.956)were independent risk factors for predicting the 28-day mortality of sepsis.The area under the curve(AUC)of the nomogram for predicting the 28-day mortality of sepsis was 0.826(95%CI=0.750-0.887),with a specificity of 78.65%,a sensitivity of 75.61%,and a Youden index of 0.543.Conclusion A nomogram created based on coagulation function indicators has certain predictive value for the 28-day mortality of sepsis,which can facilitate medical staff to take targeted medical interventions.
