The correlation of platelet,fibrinogen and lymphocyte ratio with the clinical prognosis of diffuse large B cell lymphoma

Abstract:Objective To explore the correlation of platelet(PLT),fibrinogen(FIB)and lymphocyte(LYM)ratio with the clinical prognosis of diffuse large B cell lymphoma(DLBCL).Methods A total of 86 DLBCL patients from January 2019 to October 2021 were included in the study.The levels of PLT,LYM and FIB were detected on admission,and the PLT/LYM ratio(PLR),and FIB/LYM ratio(FLR)were calculated.According to the clinical prognosis,patients were divided into the survival group and mortality group.The correlation of PLR,and FLR with the clinical prognosis of DLBCL was analyzed by the point two column correlation and restricted cubic spline model.The receiver operating characteristic(ROC)curve was drawn to test the predictive value of PLR and FLR in the clinical prognosis of DLBCL.The decision curve was drawn to evaluate the clinical benefit of PLR and FLR in predicting the clinical prognosis of DLBCL.Results A total of 86 patients with DLBCL were included in this study.Two patients were excluded,and 84 patients completed the study.Among the 84 patients,19(22.62%)died of illness were included in the mortality group,while the remaining 65 patients were included in the survival group.Through point two column correlation analysis,PLT,FIB,PLR,and FLR were all positively correlated with the mortality of DLBCL(P<0.05),while LYM was negatively correlated with the mortality of DLBCL(P<0.05).According to the analysis of the restricted cubic spline model,the correlation between PLT,LYM,FIB,PLR,and FLR showed a linear dose-response relationship with the mortality of DLBCL patients(P<0.05).ROC curve showed that PLR,and FLR alone and their combination had certain predictive value for the death of DLBCL patients,and the combined predictive value was the highest(P<0.05).The decision curve showed that the net benefit rate of PLR combined with FLR in predicting the death of DLBCL patients was significantly better than that of single index within the threshold range of 0.03-0.44、0.61-1.00,and the maximum net benefit rate was 0.226.Within the range of high-risk threshold of 0.04-1.00,the net benefit rate of combined prediction was>0,which was of clinical significance.Conclusion PLR and FLR are closely related to the clinical prognosis of DLBCL,and can be used as effective indicators to predict the clinical prognosis of DLBCL.The combination of the two indicators can improve the predictive efficiency.
