Clinical application value of conventional radiotherapy versus large fraction radiotherapy for early-stage breast cancer patients undergoing breast conserving surgery

Abstract:Objective To compare the clinical value of conventional radiotherapy versus large fraction radiotherapy for early-stage breast cancer patients treated with breast conserving surgery.Methods A total of 120 patients with early-stage breast cancer who received breast conserving surgery from August 2021 to August 2022 were selected.They were randomly divided into the control group and observation group,with 60 patients in each group.Patients in the control group received conventional radiotherapy after surgery,and those in the observation group received large fraction radiotherapy.T lymphocyte subset indicators(CD3+,CD4+,CD8+,CD4+/CD8+),tumor marker levels(carcinoembryonic antigen,carbohydrate antigen 15-3,alpha fetoprotein),incidence of adverse reactions,and dose to organs at risk before and after radiotherapy were compared between groups.Results Before radiotherapy,there were no significant differences in the levels of T lymphocyte subsets and tumor markers between the two groups(P>0.05).After radiotherapy,compared with the control group,patients in the observation group had significantly higher levels of CD3+,CD4+,CD4+/CD8+,but lower levels of CD8+(P<0.05).Significantly lower levels of carcinoembryonic antigen,carbohydrate antigen 15-3,and alpha fetoprotein were seen in the observation group than the control group after treatment(P<0.05).The overall incidence of adverse events was significantly lower in the observation group compared to the control group(6.67%vs 20.00%,P<0.05).The dose to organs at risk in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Large fraction radiotherapy after breast conserving surgery for early-stage breast cancer has positive value,which can effectively improve the immune function,prevent or reduce adverse events,and effectively reduce the radiation dose of crisis organs.Compared with conventional radiotherapy,it is safer and more effective.
