Correlation of serum fibrinogen,prealbumin,cholinesterase,and pulmonary inflammation index on chest computed tomography with the severity and prognosis of pneumonia

Abstract:Objective To investigate the correlation of fibrinogen(FIB),prealbumin(PA),cholinesterase(ChE),and pulmonary inflammation index(PII)on chest computed tomography(CT)with the severity and prognosis of pneumonia.Methods A total of 220 adult patients with community-acquired pneumonia who received treatment from January 2022 to June 2023 were selected as the study objects.They were divided into the severe pneumonia group(n=75)and non-severe pneumonia group(n=145).According to the 30-day prognosis,they were divided into the survival group(n=185)and death group(n=35).Serum FIB,PA,ChE levels and PII on chest CT were collected and compared.SPSS27.0 was used to analyze the relationship between each index and the severity of pneumonia.Receiver operating characteristic(ROC)curve was drawn by MedCal 22.0 software to analyze the predictive value of FIB,PA,ChE and PII in diagnosing pneumonia.Results FIB and PII in severe pneumonia group were significantly higher than those of the non-severe pneumonia group,and PA and ChE were significantly lower(P<0.05).FIB and PII in the death group were significantly higher than those of the survival group,and PA and ChE in the death group were significantly lower(P<0.05).Pearson linear correlation analysis showed that FIB and PII were positively correlated with the Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ)score(P<0.05),while PA and ChE were negatively correlated with it(P<0.05).ROC curve analysis indicated that FIB,PA,ChE and PII had high sensitivity and specificity in predicting death of pneumonia patients,and their combination had the highest value in predicting death of pneumonia patients.Conclusion FIB and PII increase,and PA and ChE decrease with the increased severity of pneumonia.FIB,PA,ChE and PII all have certain prognostic value in patients with pneumonia,and their combination has the highest prognostic value in patients with pneumonia.
