Potential of quantitative late gadolinium enhancement cardiac magnetic resonance in assessing the prognosis of Non-ischemic cardiomyopathy

Abstract:Objective To investigate the potential of quantitative late gadolinium enhancement(LGE)cardiac magnetic resonance in assessing the prognosis of non-ischemic cardiomyopathy.Methods The study included 68 patients with non-ischemic cardiomyopathy admitted in the Taicang Hospital of Traditional Chinese Medicine from January 2019 to January 2022.All patients underwent quantitative LGE cardiac magnetic resonance.Follow-up visits were conducted every three months via phone contact or clinic visits until May 2024.Based on the prognosis during the follow-up period,patients were divided into two groups:27 patients with adverse cardiovascular events and deaths were categorized into the poor prognosis group,while the remaining 41 patients were included in the good prognosis group.Clinical data,electrocardiographic parameters,and the extent of gadolinium enhancement were compared between the groups to explore their predictive value.Results There were no significant differences between the two groups in the age,gender,body mass index(BMI),medical history,comorbidities,heart rate,atrioventricular block,left bundle branch block,and right bundle branch block(P>0.05).Patients in the poor prognosis group had significantly longer QRS durations than those of the good prognosis group(P<0.05).There was no difference in the location of LGE between groups(P>0.05).However,the positive rate of LGE and the proportion of myocardial involvement in the enhancement pattern were significantly higher in the poor prognosis group than those of the good prognosis group(P<0.05).Multivariate logistic regression analysis showed that both QRS duration and LGE were predictive of a poor prognosis,with the area under the curve(AUC)of 0.850 and 0.755,respectively.Conclusion Quantitative LGE magnetic resonance has a high predictive efficacy for the prognosis of non-ischemic cardiomyopathy.Specifically,QRS duration and the positive rate of LGE are strong predictors of outcomes.
