Effect of mitral valve replacement and mitral valvuloplasty on postoperative regurgitation in patients with rheumatic mitral valve heart disease

Abstract：Objective To compare the effects of mitral valve replacement and mitral valvuloplasty on the postoperative reflux in patients with rheumatic mitral valve heart disease.Methods A total of 76 patients with mitral valve heart disease admitted from October 2022 to October 2023 were selected.They were divided into the group A(mitral valve replacement)and group B(mitral valvuloplasty)according to different treatment methods,with 38 cases per group.The postoperative reflux,operation-related indexes,complications and cardiac function were compared between the two groups.Results There was no significant difference in mitral regurgitation grade between the two groups at 7 days postoperatively(P＞0.05).In the group B,the operation time and cardiopulmonary bypass time were significantly longer than those of group A(P＜0.05).The intraoperative blood loss was significantly less in the group B than group A,and the respiratory assistance time and hospital stay were significantly shorter than group A(P＜0.05).In group B,the incidence of complications was significantly lower than group A(5.26％vs 21.05％,P＜0.05).In group B,the left ventricular end-diastolic diameter(LVEDD)and left ventricular end-diastolic volume(LVEDV)at 7 days postoperatively were significantly lower than those of group A,and the left ventricular ejection fraction(LVEF)was significantly higher(P＜0.05).Conclusion In the treatment of rheumatic mitral valve heart disease,mitral valve replacement and mitral valvuloplasty can both effectively improve mitral valve regurgitation.Although mitral valvuloplasty is time-consuming and requires a longer period of extracorporeal circulation,it reaches less intraoperative blood loss,respiratory assistance,hospital stay,and complications,which is more conducive to improving the short-term cardiac function indicators.
