Effect of sitagliptin combined with metformin on glucose and lipid metabolism,C-peptide,body mass index and blood pressure in elderly patients with type 2 diabetes mellitus

Abstract:Objective To analyze the effect of sitagliptin combined with metformin on glucose and lipid metabolism,C-peptide,body mass index(BMI)and blood pressure in elderly patients with type 2 diabetes mellitus(T2DM).Methods The study cases included elderly T2DM patients without serious acute or chronic complications,with a visit time range from June 2019 to December 2022.According to the order of visits,they were divided into Group A(treated with sitagliptin combined with metformin,120 cases)and Group B(treated with metformin,100 cases).The blood glucose-related indicators,C-peptide,BMI,blood lipid indicators,blood pressure before and 6 months after treatment were included as comparisons,and the incidence of treatment-emergent adverse events(TEVEs)was assessed.Results There was no statistically significant difference in fasting blood glucose,2-hour postprandial blood glucose,glycated hemoglobin,fasting C-peptide,and 2-hour postprandial C-peptide between groups before treatment(P>0.05).After 6 months of treatment,fasting blood glucose([7.35±1.66]mmol/L vs[8.09±1.93]mmol/L)and 2-hour postprandial blood glucose([8.52±1.78]mmol/L vs[9.23±1.89]mmol/L),glycated hemoglobin([5.63±1.03]% vs[6.07±1.17]%)in Group A were significantly lower compared to Group B(P<0.05),while fasting C-peptide([1.98±0.59]ng/mL vs[1.63±0.67]ng/mL)and 2-hour postprandial C-peptide([7.93±1.38]ng/mL vs[6.16±1.25]ng/mL)were significantly higher(P<0.05).There was no statistically significant difference in body mass index,blood lipid indicators,and blood pressure levels between groups before treatment(P>0.05).After 6 months of treatment,Group A had significantly lower body mass index,triglyceride(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),and blood pressure levels(P<0.05),and higher high-density lipoprotein cholesterol(HDL-C)compared to Group B(P<0.05).The incidence of TEVEs in Group A was comparable to Group B(22.50%vs 25.00%,P>0.05).Conclusion The use of sitagliptin combined with metformin in the treatment of elderly T2DM patients can effectively improve islet beta cells function,enhance blood sugar control,improve lipid metabolism,reduce body mass index,and control blood pressure levels,with a high drug safety.
