Correlation of serum 25(OH)D and CHI3L1 with elderly T2DM patients with sarcopenia

Abstract:Objective To explore the predictive values of serum 25-hydroxy vitamin D[25(OH)D]and chitinase-3-like protein 1(CHI3L1)in elderly type 2 diabetes mellitus(T2DM)patients combined with sarcopenia.Methods From January 2023 to April 2024,a total of 193 elderly T2DM patients were divided into sarcopenia group(67 cases)and non-sarcopenia group(126 cases)based on sarcopenia criteria.Serum 25(OH)D levels were measured by chemiluminescent immunoassay,and serum CHI3L1 levels were determined by enzyme-linked immunosorbent assay(ELISA).Logistic regression was used to identify independent predictors of sarcopenia.Receiver operating characteristic(ROC)curves were plotted,and the area under the curve(AUC)was evaluated to assess the diagnostic performance of serum 25(OH)D and CHI3L1 in elderly T2DM patients with sarcopenia.Results Age,serum 25(OH)D levels,and serum CHI3L1 levels were influencing factors for sarcopenia in elderly T2DM patients.Correlation analysis showed that serum 25(OH)D was positively correlated with the appendicular skeletal muscle mass index(ASMI),and negatively correlated with age and serum CHI3L1.The diagnostic performance of serum CHI3L1 levels was significantly higher than that of age(Z=4.136,P<0.05).Further analysis revealed that the diagnostic performance of combined detection of serum 25(OH)D levels and CHI3L1 levels was significantly higher than that of age,serum 25(OH)D levels,and CHI3L1 levels alone(Z=4.989,5.899,and 2.609,P<0.05).Conclusion Serum 25(OH)D levels and CHI3L1 levels can effectively predict sarcopenia in elderly T2DM patients,serving as auxiliary diagnostic indicators for an early diagnosis of sarcopenia.
