Correlation between preoperative stone volume and mean CT value of stones with surgical efficiency in ureteroscopic holmium laser lithotripsy

Abstract：Objective To analyze the correlation between preoperative stone volume and mean CT value of stones with surgical efficiency of ureteroscopic holmium laser lithotripsy.Methods One hundred patients with ureteral calculi admitted from July 2022 to December 2022 were included,and all of them underwent ureteroscopic holmium laser lithotripsy.According to the preoperative stone volume,the patients were divided into small volume group(stone volume＜700 m3,n=57)and large volume group(stone volume ≥ 700 m3,n=43).According to the difference in preoperative CT value,patients were assigned into low CT value group(CT value＜1000 Hu,n=52)and high CT value group(CT value ≥ 1000 Hu,n=48).The operation situation,and stone clearance rate,and the incidence of postoperative complications were compared between groups.The correlation between preoperative stone volume,CT value of stone,operation time with stone clearance rate was analyzed.The independent risk factor that affected the clearance rate of ureteroscopic holmium laser lithotripsy was assessed.Results Compared with the large-volume stone group,the small-volume stone group had significantly less intraoperative bleeding,significantly higher single stone clearance rate and the proportion of simple stones,and significantly shorter operation time and hospitalization time(P＜0.05).The incidence of postoperative complications in small volume group was significantly lower than that of large volume group(12.28％vs 30.23％,P＜0.05).Compared with the high-CT group,the patients in the low-CT group presented significantly less intraoperative bleeding,significantly higher single stone clearance rate and the proportion of simple stones,and significantly shorter operation time and hospitalization time(P＜0.05).The incidence of postoperative complications in low CT value group was significantly lower than that of high CT value group(11.54％vs 29.17％,P＜0.05).The results of linear correlation analysis showed that there was a significant positive correlation between preoperative stone volume and stone clearance rate(r=0.587,P＜0.001),and between preoperative CT value and stone clearance rate(r=0.766,P＜0.001).Multivariate Logistic regression analysis proved that preoperative stone volume and preoperative CT value were independent risk factors for the clearance rate of ureteroscopic holmium laser lithotripsy(P＜0.05).Conclusion Both preoperative stone volume and mean CT value of stones are positively correlated with surgical efficiency of ureteroscopic holmium laser lithotripsy.The larger preoperative stone volume and the higher the CT value of stones are associated with the lower surgical efficiency,which are the independent risk factors for stone retention.
