Predictive value of thromboelastogram combined with the detection of peripheral blood NSE,CRP and D-dimer in predicting venous thromboembolism events after radical surgery for lung cancer

Abstract:Objective To explore the predictive value of thromboelastogram(TEG)combined with the detection of peripheral blood neuron specific enolase(NSE),C-reactive protein(CRP)and D-dimer(D-D)for predicting venous thromboembolism(VTE)events after radical surgery for lung cancer.Methods A total of 300 patients undergoing radical surgery for lung cancer were enrolled as the research objects between January 2020 and December 2022.According to presence or absence of VTE within 1 month after surgery,they were divided into VTE group(84 cases)and non-VTE group(216 cases).TEG parameters,peripheral blood levels of NSE,CRP and D-D were detected,and their predictive value for VTE was analyzed.Results Compared with those of non-VTE group,the maximum amplitude(MA)and α angle were significantly higher,while K(time taken to achieve a certain level of clot strength)and R(time of latency from start of test to initial fibrin formation)were significantly lower in VTE group(P<0.05).Compared with those of non-VTE group,peripheral blood levels of NSE,CRP and D-D were significantly higher in VTE group(P<0.05).The receiver operating characteristic(ROC)curves showed that the area under the curve(AUC)of MA,K,R,α,NSE,CRP and D-D for predicting VTE after radical surgery for lung cancer were 0.762,0.787,0.682,0.775,0.716,0.786 and 0.818,respectively,with the cut-off values of 78.52mm,2.27min,5.30min,82.25°,15.94μg/L,20.12mg/L and 3.64mg/L,respectively.The AUC of the combined detection of the six indexes was 0.968,which was significantly larger than that of a single detection(P<0.05).Conclusion TEG parameters combined with the detection of peripheral blood NSE,CRP and D-D have high a predictive value for VTE events after radical surgery for lung cancer.
