Study on the curative effects and radiation toxic and side reactions on intensity modulated radiation therapy in treatment of esophageal carcinoma

Abstract:Objective To investigate the efficacy, short-term prognosis and radiation toxic and side reactions of intensity modulated radiation therapy ( IMRT ) in treatment of esophageal carcinoma. Methods Eighty patients with esophageal carcinoma who were admitted and treated in our hospital from January 2012 to December 2014 were enrolled in the study. According to random number table,these patients were divided into 3DCRT group and IMRT group ,with 40 patients in each group. The patients in 3DCRT group were treated by 3D conformal radiation therapy,however,the patients in IMRT group were treated by IMRT. At the end of treatment,the clinical curative effects, incidence rates of toxic and side effects were observed and compared between two groups. Moreover the irradiation dose of planning target volume (PTV),clinical target volume (CTV),gross target volume (GTV) and organ at risk (OAR) were also observed and compared between two groups. All the patients were followed up for 24 months,then the survival rate was compared between two groups. Results The target volume of minimum value and mean value of the PTV,CTV and GTV in IMRT group were significantly higher than those in 3DCRT group, however the OAR of maximum volume and mean volume of PTV, CTV and GTV in IMRT group were significantly lower than those in 3DCRT group (P<0. 05). On 1 month after treatment,there were no significant differences in objective remission rate and clinical control rate between two groups (P > 0. 05),but incidence rates of bone marrow suppression and radiation pneumonitis in IMRT group were significantly lower than those in 3DCRT group (P>0. 05). After 24-month follow up,there were no significant differences in 1-year survival rate and 2-year survival rate between two groups (P>0.05).Conclusion Theclinicalcurativeeffectsandshort-termprognosisofIMRTintreatmentofesophagealcancerare similar to those of 3DCRT,but the dose advantage of IMRT is superior to that of 3DCRT,moreover, which can reduce the incidence rates of toxic and side effects,therefor, which is worthy of clinical attention.
