Therapeutic effects of levosimendan combined with trimetazidine on ischemic cardiomyopathy in the elderly

Abstract：Objective To investigate the therapeutic effects of levosimendan combined with trimetazidine on ischemic cardiomyopathy in the elderly. Methods A total of 106 elderly patients with ischemic cardiomyopathy complicated by heart failure who were treated in our hospital from March 2016 to March 2017 were randomly divided into control group and observation group,with 53 patients in each group. The patients in both groups were treated by routine cardiotonics,diuresis and dilating blood vessels,on this basis,the patients in control group were treated by levosimendan injections,however,the patients in observation group were treated by levosimendan combined with trimetazidine. After treatment the curative effects,cardiac function, myocardial injury markers levels, rehospitalization rate, mortality and adverse drug reactions were observed and compared between two groups. Results The effective rate in observation group was 94. 34% ,which was significantly higher than that (77. 36% ) in control group (P＜0. 05). Before treatment there were no significant differences in LVESD,LVEDD, LVEF,and 6MWT between two groups (P＞0. 05). After treatment, LVESD,LVEDD,LVEF and 6MWT in observation group were significantly improved,as compared with those in control group (P＜0. 05). Before treatment there were no significant differences in NT-proBNP,hsCRP and urine volume between two groups (P＞0. 05). After treatment, NT-proBNP,hsCRP and urine volume in observation group were significantly decreased,as compared with those in control group (P＜0. 05). Within 6-month follow up, the rehospitalization rate in observation group was 30. 19% and the mortality was 7. 55% ,which were significantly lower than those (52. 83% and 20. 75%) in control group (P ＜0. 05). Moreover there were no significant differences in the incidence rates of adverse reactions including dry mouth,dizziness,gastrointestinal reaction,hypotension, ventricular tachycardia between two groups (P ＞0. 05). Conclusion The levosimendan combined with trimetazidine has obvious therapeutic effects on ischemic cardiomyopathy complicated by heart failure in the elferly, which can effectively improve heart function,protect myocardial cells,inhibit abnormal metabolism,delay ventricular remodeling,with less adverse reactions.
