The main cause and evaluation of cardiac function of the elderly inpatient with heart failure in middle range of ejection fraction

Abstract:Objective To analyze the clinical indexes, etiology and cardiac function of elderly inpatients with heart failure whose ejection fraction is in middle range. Methods According to left ventricular ejection fraction ( LVEF) levels, 189 elderly patients with heart failure were divided into HFrEF grpup ( LVEF<40%) , HFrmEF group ( LVEF 41% ~49%) and HFpEF group ( LVEF>50%) . The social demographic data, NYHA classification, etiology, cardiac ultrasound indexes, blood routine examination and blood biochemical indexes were observed and compared among the three groups. Results There were significant differences in the proportion of NYHA classⅡ, systolic blood pressure, LVEF levels between HFrmEF group and HFrEF group, HFpEF group(P<0. 05). The proportion of ischemic heart disease and hypertensive heart disease was higher in in HFpEF group and HFrmEF group, which was significantly higher than that in HFrEF group(P<0. 05). The serum levels of triacylglycerol and uric acid in HFrmEF group were significantly higher than those in HFrEF group ( P <0. 05), moreover, which were increased with the decrease of LVEF level, however, the levels of glutamate pyruvate transaminase,glutamic-oxal( o) acetic transaminase, homocysteine, BNP and NT-proBNP were significantly lower than those in HFrEF group (P <0. 05). The left atrium inner diameter and left ventricular inner diameter in HFrmEF group were significantly lower than those in HFrEF group, however, which were significantly higher than those in HFpEF group ( P <0. 05). Conclusion The heart function in HFrmEF group is mainly NYHA Ⅱ and Ⅲ, and ischemic heart disease and hypertensive heart disease are the main causes of disease. There are significant differences in the levels of triacylglycerol, blood uric acid, glutamate pyruvate transaminase, glutamic-oxal ( o ) acetic transaminase, homocysteine, BNP and NT-proBNP between HFrmEF group and HFrEF group as well as HFpEF group.
