Effects of full thoracoscopic anatomic lung segment resection and lobectomy on postoperative pulmonary function in patients with non-small cell carcinoma

Abstract:Objective To investigate the specific effects of anatomical lung segment resection and lobectomy under video-assisted thoracoscopic surgery ( VATS) on postoperative pulmonary function in patients with non-small cell carcinoma ( NSCLC) in order to provide theoretical basis for the popularization and application of VATS technique. Methods Sixty-two patients with NSCLC were divided into two groups according to the results of preoperative examination. The patients in experimental group were treated by VATS method for anatomical lung segment resection,and the patients in control group wre treated by VATS method for lobectomy. The pulmonary function indexes including FEV1%, FVC%, MVV% and lung volume indexes including DLCO%, TLC, RV, RV/TLC were observed and compared between two groups. Results There were significant differences in FEV1, FVC and MVV between the anteroposterior and posterior side in both groups (P<0. 05), moreover there were significant differences in FEV1,FVC and MVV in posterior side between the two groups (P<0. 05). As compared with those in control group, the levels of DLCO, TLC, RV, IC, RV/RV were significantly increased in experimental group (P<0. 05). In addition there were significant differences in DLCO, TLC, RV, IC and RV/TLC between the anteroposterior and posterior side in experimental group (P <0. 05),moreover, there were significant differences in DLCO, TLC, RV, IC and RV/TLC between the anteroposterior and posterior side in control group (P<0. 05). Besides there were significant differences in DLCO, TLC, RV, IC and RV/TLC in posterior side between two groups (P <0. 05). Conclusion The full thoracoscopic anatomic lung segment resection and lobectomy in treatment of NSCLC has a certain adverse effects on pulmonary function and lung volume of patients, however, to take anatomical lung segment resection according to the scope and location of the lesion is beneficial to reserve maximatily the pulmonary function and lung volume of patients.
