Changes of bone mineral density and serum levels of 25 (OH) D in female patients with hyperthyroidism Change of bone mineral density and 25(OH)D in female patients with hyperthyroidism
Abstract:Objective To observe the effects of thyroid function on bone mineral density and changes in the levels of serum 25 (OH) D in female patients with hyperthyroidism. Methods A total of 124 women were divided into hyperthyroidism group (n =64),which was subdivided into premenopausal hyperthyroidism group (n =30) and postmenopausal hyperthyroidism group (n =34),and control group (n =60),which was subdivided into premenopausal control group (n =28) and postmenopausal control group (n =30).All patients underwent natural menopause.Double-energy X-ray absorptiometry was used to measure bone mineral density in lumbar vertebrae (L2 ~ L4) and proximal femurs (femoral neck, greater trochanter, intertrochanteric region and Ward's triange region). The bone density in hyperthyroidism group and control group was recorded for statistical analysis.Fasting blood was collected from both patients with hyperthyroidism and healthy controls, then the serum levels of FT3,FT4,TSH and 25 (OH) D were detected.Height and weight were measured and body mass indexwas calculated. Results Compared with those in control group, the serum levels of FT3 and FT4 in hyperthyroidism group were significantly increased and TSH levels were significantly decreased (P < 0.05).The serum levels of 25 (OH) D in women with hyperthyroidism were significantly higher than those in control group (P < 0.05).The bone mineral density in the lumbar vertebrae (L2 ~ L4),femoral neck,Ward's triangle region, greater trochanter region, and intertrochanter region was reduced in hyperthyroidism group. As compared with that in premenopausal control group, the bone mineral density was significantly lower in other regions other than in the right Ward's triangle region in premenopausal hyperthyroidism group (P < 0.05).Compared with that in postmenopausal control group, the bone density was decreased significantly in lumbar vertebrae and left intertrochanter regions (P < 0.05).Bone mineral density in postmenopausal hyperthyroidism group was significantly decreased in the other regions except for the right intertrechanter regions and the left intertrochanter,as compared with that in premenopausal hyperthyroidism group. The bone density of lumbar vertebrae and the proximal femur was remarkably reduced in postmenopausal control group,as compared with that in premenopausal control group. The bone mineral density in lumbar vertebrae and proximal femur in postmenopausal hyperthyroidism group was significantly lower than that in premenopausal control group. Conclusion Hyperthyroidism can lead to bone loss and bone density decrease. Moreover, menopause can also cause bone density decrease.Specifically, the bone mineral density of menopausal women with hyperthyroidism is decreased most significantly.Nevertheless, the serum levels of 25 (OH) D in patients with hyperthyroidism are significantly increased.
