Clinical effects of different kinds of laryngoscope in tracheal intubation under general anesthesia in treatment of cervical spinal cord injury

Abstract:Objective To observe the clinical effect of different kinds of laryngoscope in tracheal intubation under general anesthesia in treatment of cervical spinal cord injury.Methods A total of 120 patients with cervical spinal cord injury were randomly divided into group A (Shikani laryngoscope,n =30),group B (GlideScope video aryngoscope,n =30),group C (McGrath5 video-laryngoscope,n =30) and group D (Macintosh direct laryngoscope,n =30).The curvature changes of cervical vertebra when glottis was exposed were observed and compared among the four groups.In addition,the levels of oxygen saturation,heart rate and mean arterial pressure at different time points were observed and compared.The success rate,intubafion time and incidence rate of complication after surgery were also observed and compared among the four groups.Results The curvature changes of cervical vertebra when glottis was exposed and after tracheal intubation in group B and group C were significantly lower than those in group D,which were significantly higher than those in group A (P < 0.05),however,there were no significant differences between group B and group C (P > 0.05).The curvature changes of cervical vertebra in group A were less than those in group B and group C (P > 0.05).There was no significant change in levels of SPO2 from T1 to T4 in the four groups (P > 0.05).The changes of MAP and HR in group B and group C were less than those in group D (P < 0.05),and there were no significant differences between group B and group C (P > 0.05).The changes of MAP and HR in group A were less than those in group B and group C (P < 0.05).The success rate in once intubation in group B and group C was significantly lower than that in group A (P <0.05),however,which was significantly higher than that in group D(P < 0.05),there was no significant difference between group B and group C (P > 0.05).The intubation time in group B and group C was longer than that in group A (P <0.05),however,which was shorter than that in group D (P < 0.05),there was no significant difference between group B and group C (P > 0.05).There was no significant difference in incidence rate of complication after intubation among the four groups (P > 0.05).Conclusion Shikani laryngoscope in treatment of cervical spinal cord injury has a higher success rate for once intubation,with little injury,lower complication and higher safety,thus,which is worth using widely in clinical practice.
