Effects of PPH combined with MMH on edema of wound margin and anal canal pressure in patients with severe circumferential mixed hemorrhoid

Abstract:Objective To investigate the effects of procedure for prolapse and hemorrhoids (PPH) combined with Millidan Morgan hemorrhoidectomy (MMH) on edema of wound margin and anal canal pressure in patients with severe circumferential mixed hemorrhoid in order to provide guidance for clinical study. Methods Ninety-six patients with circumferential mixed hemorrhoid who were treated in our hospital from May 2013 to May 2015 were randomly divided into control group(n=48) and observation group(n=48).All the patients were treated by anti-infection firstly,then the patients in control group were treated by MMH surgery,however,the patients in observation group were treated by MMH combined with PPH surgery.The patients in both groups were followed up at after 1 month after operation.The therapeutic effects,and changes of edema and anal canal pressure before and after operation and the incidence rates of adverse reactions were observed and compared between two groups. Results The operation time,intraoperative blood loss,hospitalization time and wound healing time in observation group were significantly shorter than those in control group (P<0.05).The edema scores in observation group were (5.88 ± 1.28) points,which were (2.44 ± 1.03) points in control group,and the edema degree in observation group was significantly lower than that in control group (P<0.05).The resting pressure and systolic blood pressure were significantly decreased after treatment in both groups(P<0.05),moreover,the decrease degree in observation group was more obvious than in control group (P <0. 05),however,there were no significant differences in rectal sensory capacity and maximum capacity between two groups(P>0.05). In addition the incidence rates of postoperative pain,urinary retention and post stool hemorrhage in observation group were significantly lower than those in control group(P<0.05).Conclusion PPH combined with MMH can obtain satisfactory therapeutic effects on severe circumferential mixed hemorrhoid,with better recovery,less pressures on anal tube,fewer incidencerate of postoperative complications,therefor,which is worth using widely in clinical practice.
