Retrospective analysis for the effects of dangerous central placenta previa on maternal-infant outcome
Abstract:Objective To analyze retrospectively the effects of dangerous central placenta previa on maternal-infant outcome. Methods A retrospective study was performed to investigate the types of dangerous placenta previa, and to analyze the maternal-infant outcome of three kinds of dangerous placenta previa. Results Among 45 cases of dangerous placenta previa, there were 21 cases(46.67%) of central placenta previa,11 cases(24.44%) of partial placenta previa and 13 cases (28.89%) of marginal placenta praevia. The proportion of central placenta previa was the highest, which was significantly different from that of partial placenta previa or marginal placenta previa(P<0.05). There were significant differences in the number of gestational age, gestational age, miscarriage, miscarriage and cesarean section among different types of placenta previa (P < 0. 05). Logistic multivariate regression analysis showed that age, abortion frequency and cesarean section frequency were risk factors of central dangerous placental placenta(P<0.05). The incidence rates of pregnancy badness and bad outcome in central placental placenta group were significantly higher than those in partial placental placenta group and marginal placental placenta group, and the clinical manifestations were prenatal hemorrhage, inhibiting uterine contraction treatment, postpartum hemorrhage, high placental implantation rate, high hysterectomy rate (P <0.05). There were no significant differences in incidence rates of shock, DIC and placental adhesions between the three groups (P >0. 05), however, there were significant differences in incidence rates of premature delivery and neonatal asphyxia among the three groups (P<0.05). The incidence rates of premature delivery and neonatal asphyxia in central placenta previa group were obviously higher than those in partial placenta previa group and marginal placenta previa group, but there were no significant differences in low birth weight infants and neonatal mortality among the three groups (P>0.05).Conclusion In patients with dangerous placenta previa, the incidence rate of central placenta previa is highest, moreover, the incidence rate of bad maternal-infant outcome of dangerous central placenta previa is higher, and the risk factors include pregnant woman's age, birth frequency and cesarean section frequency.
