The risk factors of poor prognosis in elderly patients with hip fracture after operation

Abstract：Objective To investigate the independent risk factors of poor prognosis in elderly patients with hip fracture after operation.Methods The clinical data about 2005 patients with hip fracture who were admitted and treated by surgery in our hospital from January 2009 to December 2016 were retrospectively analyzed,in which,the acute cerebral stroke after operation was regarded as poor prognosis index.According to whether the patients had acute cerebral stroke on 2 weeks after operation,the patients were divided into two groups: stroke group (n=48) and control group (n=1957).The various relevant indexes were analyzed and compared between two groups,moreover, the independent risk factors influencing the incidence of acute cerebral stroke in elderly patients with hip fracture after operation were analyzed by Logistic regression method.Results The male patients accounted for 67.96% and female patients accounted for 32.04% in control group, however,the male patients accounted for 41.67% and female patients accounted for 58.33% in stroke group. The female patients who had acute cerebral stroke after operation in stroke group were significantly more than those in control group(P<0.05). The patients who had more than 3 kinds of internal diseases and suffered from abnormal blood pressure in stroke group were significantly less than those in control group (P<0.05). Multiple factors analysis showed that the history of cardiac arrhythmias (OR =5. 868,P <0. 01), the history of cerebral stroke (OR =4. 261,P <0. 05) and the incidence of intraoperative blood pressure abnormality(OR=3.468,P<0.05) were the independent risk factors influencing the incidence of acute cerebral stroke in elderly patients with hip fracture after operation (P<0.05).Conclusion The independent risk factors influencing the incidence of acute cerebral stroke in elderly patients with hip fracture after operation are history of cardiac arrhythmias,the history of cerebral stroke and incidence of intraoperative blood pressure abnormality.
