Therapeutic effects and safety of two doses of gamma globulin combined with glucocorticoid on acute moderate or severe GBS in children

Abstract:Objective To investigate the therapeutic effects and safety of high dose and low dose intravenous injection of gamma globulin combined with glucocorticoid pulse therapy on acute moderate or severe Guillain Barre syndrome (GBS) in children.Methods A total of 127 children patients with acute moderate or severe GBS who were treated in our hospital from June 2015 to December 2016 were enrolled in the study.These patients were randomly divided into high dose group (n =59) and low dose group (n =68).The patients in low dose group were treated by gamma globulin 0.2g · kg-1 · d-1 combined with methylprednisolone,however,the patients in high dose group were treated by gamma globulin 0.4g · kg-1 · d-1 combined with methylprednisolone.The changes of inflammatory factors levels,drug onset time,invasive ventilation,therapeutic effects and adverse reactions before and after impact treatment (5d) were observed and compared between two groups.Results After impact treatment,the serum levels of inflammatory factors including CRP,IL-6,TNF-alpha were significantly decreased in both groups,as compared with those before impact treatment (P < 0.05),but there were no significant differences between the two groups (P >0.05).There were no significant differences in recovery of respiratory muscle paralysis,the improvement of muscle strength and recovery of sensory disturbance and the length of staying in hospital between two groups (P > 0.05).The total effective rate in low dose group was 91.18%,which was 89.83% in high dose group,there was no significant difference between two groups (P > 0.05).In aspect of adverse reaction,there were no significant differences in the incidence rates of fever,nausea,headache,palpitations and the other conditions between two groups (P > 0.05).Conclusion The high dose and low dose gamma globulin intravenous injection combined with methyl prednisone impact therapy has equivalent therapeutic effects and safety on acute moderate or severe GBS in children patients,therefor,it is suggested to use low dose drugs in order to reduce economic burden of family and medication potential risk.
