Effects of bone flap ladder decompression control technique on the serum levels of IL-6, CRP and NSE and the prognosis of patients with severe brain trauma

Abstract:Objective To investigate the effects of bone flap ladder decompression control technique on the serum levels of interleukin-6 (IL-6),C-reactive protein (CRP),neuron specific enolization E (NSE) and the prognosis of patients with severe brain trauma.Methods One hundred patients with severe brain trauma who were admitted and treated in our hospital from December 2013 to December 2016 were randomly divided into ladder control group and the standard decompression group according to the random number table,with 50 patients in each group.The patients in standard decompression group were treated by standard bone flap decompression treatment,however,the patients in ladder control group were treated by bone flap ladder decompression control technique.Then the therapeutic effects were evaluated according to clinical neural function defect scale (NFD),and the serum levels of IL-6,CRP and NSE were detected by enzyme-linked immunosorbent (ELISA).Moreover patient's life quality was evaluated by activity of daily living (ADL),and the patients were followed up for 6 months,the therapeutic effects,incidence rates of complications and death,the changes of neural function before and after the treatment,the serum levels of IL-6,CRP and NSE,the cerebral oxygen uptake rate (CEO2) before treatment and on 1 d,4d,7d after treatment and life quality before treatment and on 1 m,3 m,6m after treatment were observed and compared between the two groups.Results After treatment the NFD scores in ladder control group were significantly lower than those in standard decompression group,however,the therapeutic effects in the former were significantly higher than those in the latter (P < 0.05).The incidence rates of complication in ladder control group were significantly lower than those in standard decompression group (P < 0.05),but there were no significantly differences in death rates between two groups (P > 0.05).After treatment the serum levels of IL-6,CRP and NSE in ladder control group were significantly lower than those in standard decompression group (P <0.05),however,the CEO2 levels on 4d,7d after treatment in ladder control group were significantly higher than those in standard decompression group (P < 0.05),besides,the ADL scores on lm,3m,6m after treatment in ladder control group were significantly higher than those in standard decompression group (P < 0.05).Conclusion The bone flap ladder decompression control techniquecan can obviously erthance the therapeutic effects on brain trauma and improve the brain oxygen intake capacity of patients so as to save patient's life,which can decrease the risk of postoperative complications and promote the recovery of neurological function and life quality of patients,and its action mechanism may be related with the improvement of serum levels of IL-6,CRP and NSE,therefor,which is worth using widely in clinical practice.
