Effects of sitagliptin on oxidative stress,inflammatory reactions and peripheral neuropathy of patients with type 2 diabetes mellitus

Abstract:Objective To observe the effects of sitagliptin on oxidative stress ,inflammatory reactions and peripheral neuropathy in treatment of type 2 diabetes mellitus .Methods One hundred and fifty patients with type 2 diabetes peripheral neuropathy were randomly divided into observation group (n=75) and control group (n=75).The patients in control group were treated by insulin +metformin hydrochloride, however, the patients in observation group were treated by insulin +sitagliptin,with a treatment course of 8 weeks for both groups.The changes of oxidative stress ,inflammatory reactions,and symptoms scoring system (TSS) score,ankle brachial index (ABI) and peripheral nerve conduction velocity (NCV) before and after treatment were observed and compared between two groups .Results After treatment the levels of superoxide dismutase ( SOD ) activity were increased , however , the levels of malondialdehyde were decreased for both groups , but the improvement degree in observation group was superior to that in control group (P<0.05).After treatment the levels of serum hypersensitive C reactive protein ( hs-CRP ) and endothelin 1 ( ET 1 ) were significantly decreased in both groups , but the improvement degree in observation group was superior to that in control group (P<0.05).Moreover the levels of fasting plasma glucose (FPG), glycosylated hemoglobin (HbA1c) protein,triglyceride (TG),cholesterol (TC) were decreased after treatment in both groups , however , the improvement degree in observation group was superior to that in control group ( P<0.05).In TSS scoring system, stabbing pain, numbness, paresthesia, burning sensation and total integral were decreased after treatment, but the improvement degree in observation group was superior to that in control group (P<0.05).Besides the levels of ABI and NCV were increased after treatment for both groups , however , the levels of ABI and NCV in observation group were significantly higher than those in control group( P<0 .05).Conclusion The application of sitagliptin in treatment of type 2 diabetes mellitus can improve patients ' clinical symptoms and improve glucolipid metabolism ,and can increase ankle brachial index , enhance nerve conduction velocity , and its action mechanism may be related with its inhibitory effects on oxidative stress and inflammatory reactions .
