The research of diagnostic value of serum progranulin in patients with diabetic kidney disease

Abstract：Objective To investigate the diagnostic value of serum progranulin ( PGRN ) in patients with type 2 diabetes mellitus (T2DM) complicated by diabetic kidney disease (DKD).Methods A total of 136 patients with T2DM who were admitted and treated in our hospital from April 2011 to May 2015 were enrolled in the study .They were divided into normal urinary albumin (UmAlb) group (A1 group,n=43),moderate UmAlb group (A2 group,n=40),severe UmAlb group (A3 group,n=32) and dialysis group (n=21) according to UmAlb and dialysis condition .The general data and the routine laboratory parameters were observed .The serum levels of PGRN were detected by ELISA .The ROC curve was used to analyze the diagnostic value of of simple PGRN or PGRN combined with Csy-C for renal function impairment in patients with DKD . Results The serum levels of PGRN were in order (fron high to low) dialysis group [(993.62 ±444.39)mg/L],A3 group [(542.98 ±383.79) mg/L],A2 group [(357.04 ±272.81) mg/L],A1 group [(255.23 ±226.11) mg/L] and control group [(148.72 ±115.48) mg/L].In patients with T2DM,the levels of PGRN were positively correlated to those of ACR (r=0.601,P<0.01),DM duration (r=0.270,P=0.01),sCr (r=0.620,P<0.01) and Csy-C (r=0.723,P<0.01), however,which were negatively correlated to those of eGFR (r=-0.613,P<0.01).The AUC of simple PGRN detection in diagnosis of renal function damage was 0.802(95%CI:0.720~0.885),however, the AUC of PGRN combined with Csy-C in diagnosis of renal function damage was 0.851(95%CI:0.783-0.920).Conclusion The increase of serum levels of PGRN in patients with T2DM suggests that there exists renal functional lesion ,moreover, PGRN combined with Csy-C can be regarded as the clinical indexes to evaluate renal functional lesion of patients with DKD .
