Effects of Tripterygium glycosides combined with valsartan on the serum levels of D-dimer and inflammatory cytokines in patients with early diabetic nephropathy

Abstract：Objective To observe the effects of Tripterygium glycosides combined with valsartan on the serum levels of D-dimer ( D-D ) and inflammatory cytokines in patients with early diabetic nephropathy ( DN) .Methods Eighty patients with DN were divided into normal albuminuria group (NA group, n =45,UMA/Cr <30mg/mmol) and microalbuminuria group group (MA group, n =35,UMA/Cr 30~299mg/mmol) according to urinary microalbumin/creatinine ratio (UMA/Cr) .Moreover 30 healthy subjects were served as control group .The patients in both groups were treated by basic treatment , in addition to that , the patients in both groups were treated by Tripterygium glycosides combined with valsartan treatment ,with a treatment course of 16 weeks for both groups .The levels of fasting plasma glucose ( FPG ) , glycosylated hemoglobin (HbA1c),total cholesterol (TC),triglyceride (TG),low density lipoprotein cholesterol (LDL-C),high density lipoprotein cholesterol (HDL-C), plasma glucose 2 hours after meal (2h PG),creatinine (Cr),blood urea nitrogen (BUN) and D-dimer were detected by immunohistochemistry .Results As compared with those in control group ,the levels of D-D in NA group and MA group were significantly increased ,moreover, which in MA group were significantly higher than those in NA group ( P <0.05).The levels of FPG,HbA1c,TC,TG,LDL-C,2hPG,D-D were significantly decreased in NA group and MA group before and after treatment ( P <0.05),however, the levels of HDL-C were significantly increased,there were significant differences between two groups ( P <0.05).After treatment the levels of TNF-α,IL-6 and IL-18 were significantly decreased in both groups ( P <0.05).Conclusion The levels of D-D in patients with early DN are increased.Tripterygium glycosides combined with valsartan can improve microcirculation ,inhibit inflammatory cytokines and prevent micro-thrombosis,moreover, which can decrease the levels of D-D in patients with early DN .
