The short-term effects of spironolactone on left ventricular remodeling and cardiac function in patients with acute myocardial infarction

Abstract:Objective To investigate the short-term effects of spironolactone on left ventricular remodeling and cardiac function in patients with acute myocardial infarction ( AMI ) in order to provide reference for clinical treatment . Methods Seventy patients with AMI who were admitted and treated in our hospital from January 2012 to December 2014 were divided into onservation group and control group according to random number table , with 35 patients in each group .The patients in control group were treated by routine drugs ,however ,the patients in observation group ,on the basis of control group , were treated by spironolactone .After 6-month treatment ,the cardiac function ,left ventricular hemodynamics ,cardiac remodeling and incidence rates of cardiac events were observed and compared between two groups .Results After treatment,the levels of LVEDD,LVMI,LVESV,LVEDV and LA in observation group were significantly lower than those in control group ,however, the levels of IVST and LVPWT were significantly higher than those in control group ( P <0.05).The BNP,Tei index,Vop,Vp in observation group were significantly lower than those in control group , but the levels of LVEF were significantly higher than those in control group ( P <0.05).Moreover the incidence rate of arrhythmia was 14.29% in observation group,which was significantly lower than that (34.29%) in control group ( P <0.05).Conclusion The spironolactone on the basis of routine therapy,can significantly improve the recent cardiac function and left ventricular remodeling of patients with AMI , and can reduce the incidence rates of cardiac events ,therefor,which should be paid much attention to in clinical practice .
