Therapeutic effects of small needle knife on knee osteoarthritis and its effects on the expression levels of TNF-α,MMPs in joint synovial fluid of patients

Abstract：Objective To observe the therapeutic effects of small needle knife on knee osteoarthritis ( KOA) and its effects on the expression levels of tumor necrosis factor (TNF-α), matrix metalloproteinases (MMPs) in joint synovial fluidof patients.Methods Seventy -nine patients with KOA were randomly divided into small needle knife group ( n =36) and control group ( n =33 ) .The patients in small needle knife group were treated by small needle knife ,however ,the patients in control group were treated by intraarticular injection with sodium hyaluronate , with a treatment course of 4 weeks for both groups.The levels of TNF-α,MMP-3,MMP-9,MMP-13 in joint synovial fluid of patients were detected before treatment and on 1 week after treatment , moreover , the therapeutic effects and the improvement status of symptoms and signs of patients was evaluated by using Western Ontario and McMaster Universities Osteoarthritis Scale Survey ( WOMAC rating scale ) .Results At the end of treatment,the levels of TNF-α,MMP-3,MMP-9,MMP-13 in joint synovial fluid of patients and WOMAC scores were significantly decreased in both groups ,as compared with those before treatment ( P <0.05),moreover, the decrease degree in small needle knife group was significantly lower than that in control group ( P <0.05).The clinical control rate and total effective rate in small needle knife group were significantly higher than those in control group ( P <0.05).Conclusion The research results suggest that small needle knife can inhibit the release of inflammatory cytokines -TNF-α, and can decrease the expressions of MMPs ,inhibit the degradation of cartilage matrix and protect the cartilage cells so as to carry out the therapeutic effects on KOA .
